Safety

Arrive 10 minutes prior to start time of class and designate a responsible adult to supervise your child prior to and immediately his/her Creativity Soccer Pro class.

1. Absolutely no toys or snacks out of their containers during class.
2. Children MUST wear sneakers in class or they will not be allowed to participate.
3. Creativity Soccer Pro will provide a T-shirt for each child, and we ask that all children wear them to class. Extra T-shirts are available ($15 each) Sweat/track pants, shorts are recommended.
4. No sibling is allowed to take part unless they are registered.

Rain Policy

1. If rain, snow, winds are in the forecast, call 1 hour prior to your class time if you don’t receive an email update.
2. If we need to cancel your class due to rain, there will be a cancellation message, via email or phone text that we have on file.
3. Classes missed due to rain are made up by Creativity Soccer Pro before the end date of the season.

Make-up/Refund

1. If you will miss a class for a personal reason. Please notify us at least a day before (via email) in order to be eligible for make-up. Two make-up are permitted during a season contingent to availability. You must complete make-ups in the same season they were missed. No refund for miss classes.
2. Creativity Soccer Pro reserves the right to postponed a season for many reason. Don’t worry, if that happens you will be contacted via phone or email with the new starting date and your registration will be valid for the new date or automatically for the following season. If a parent decide to cancel a season after thirty days of registration for any reason, if the season has not started yet, he/she will receive a certificate which can be redeemed for any upcoming Creativity Soccer Pro activity. In the event the season has already started, there will be absolutely no refund nor certificate.
3. ​If  you paying by check, this check should be made payable to Creativity Soccer Pro.

Limitation Of Liability

My signature indicate that I give specific consent to my child’s participation in all Creativity Soccer Pro activities and believe that my child to be qualified, good health, and in proper physical condition to participate in such activities. I understand that there are risks of bodily contact injury that might be involve in these activities that are conducted by Creativity Soccer Pro. Further, I agree to release and indemnify, that is, to hold harmless in all respects Creativity Soccer Pro, their respective administrators, coaches, volunteers, agents, employees with regard to any claims, actions, damages, liabilities, loses, cost and expenses that may be incurred as a result of my child’s participation in any Creativity Soccer Pro activities. I hereby consent to and authorize Creativity Soccer Pro the rights to publish, reproduce and use for advertising or any other purposes, any photographs, video image, audio recording or other likeness of my child, me, or the guardian I have designated, and waive all rights to compensation and other rights which may arise as a result.

Updates to this policy

We may occasionally update this policy. If we make significant changes, we will notify clients in advance of the changes through our email list or through other means, such as text message. We encourage clients to periodically review this notice for the latest information on our privacy practices.
